Overview

Holistic healthcare providers often struggle to translate care plans into day-to-day actions that patients can follow. Patients, meanwhile, feel disconnected from their progress, overwhelmed by scattered advice, and uncertain about whether their activities are making an impact. By providing a comprehensive care program that combines in-person care with an intuitive app that helps patients track and improve their health, Apollo 360 Health aims to change how people approach healthcare.

The Challenge

Design an iOS app that empowers patients to set clear goals, track their targets, and follow personalized plans, while staying connected to their care team.

Solution Overview

The app is structured around the GTP Framework:

  • Goals: Long-term health outcomes (e.g., lose 55 lbs, reduce resting heart rate).
  • Targets: Quantifiable metrics from connected devices (weight, HR, glucose, sleep).
  • Plans: Personalized activity plans across fitness, nutrition, and mental wellbeing, co-created with care teams.
Blah

In order to help our team align on how a user joins and onboards into the Apollo ecosystem, we created a user flow that identifies the key points that a user needs to be aware of as well as transactional cues like notifications, emails, and phone calls that drive a user to action.

Command Center

Home & Feed

The Home screen serves as the starting point for the Apollo app by bringing the most relevant information for a patient each day. Additionally, this screen provides a frictionless area for engagement and exploration by providing a feed of recommended activities and content for a patient. This includes a snapshot of current goal progress, and a daily agenda that shows next appointments and upcoming activities.

Goal

GTP: The core of the Apollo app

The Plan section is the most important section of the app for an Apollo patient working on their health goals. Within this section, they can see their overall health goal, messages from their doctor, and how they are progressing in their targets and plan to achieve their health goal.

Goal Target Planning Diagram

Diagram that represents how a patient's plan feeds into their target that then influences their health goal.

Each of the patient’s targets is represented by a metric tied to their device. This enables a patient to keep track of their target in one place. Along with their target is an activity plan that lists activities that they have discussed with their care team and includes personal activities as well as classes at Apollo’s in person facility as well as classes within the Apollo content library.

In addition to allowing a patient to monitor and track their goals, they can also directly record activity into the app through the menu or the record activity button that is displayed prominently within the Goal section.

Quantitative Tracking

Metrics

The Metrics section displays relevant data from connected devices like Fitbit, Apple Watch, glucose monitor, or other devices. Within each Metric is other context layered in to help explain and support a patient’s path towards better health.

Health on the Go

Content Library

A rich content library supports their patients’ health goals. The videos, quizzes, recipes, and other content are curated by a patient’s care team, but a patient can also freely explore activities by category or media format.

Profile and Settings

A patient’s care team is what elevates the content and experience of Apollo above and beyond a typical recommendation system. The care team recommends information and is available to answer questions.

Spreading the Word

Marketing and Communication

In order to begin promoting this newly launched organization, I was asked to use their brand guidelines to create a marketing site, social media templates, brochure, poster, and email templates. Although their brand guidelines were very simple, the “wall of green”, secondary colors, and typography provided a jumping off point to many variations that all live under the Apollo 360 Health look and feel.

Our initial landing page was devoted to the showing the most important aspects of Apollo: the GTP program, the state of the art location and community, and the app that helps users achieve their health goals inside and outside of their appointments.

Marketing site homepage

Within the marketing site, we structured the home page as a series of modules. Some of these modules are flexible generic components that can work with many different types of content and others were specialized for a certain fixed use.

These modules were organized as content types in ExpressionEngine, our CMS of choice, and could be pulled into a page to create custom content.

Social Media Library

To spread the word on social media, I created a series of posts and then set them up in Canva so that the Apollo team, who did not have a full time designer or social media coordinator, would be able to easily create visually interesting content that stays on brand.

Additionally, I created simple video templates that could be used for video content. These were made in After Effects and exported as MGRTs so that they could be easily edited as templates in Premiere.

Email Template

Continuing with the theme of marketing templates, I also designed an email template for Apollo to deploy in Salesforce.
